Provincial Examinations
Forms and Instructions
The following forms must be submitted to the Ministry for students requiring adaptations to exam conditions:
Reader or Scribe
- The form Request for Adjudication (Appendix 2A)[122 kb]
, along with supporting documentation, must be completed and sent to the Ministry by the adjudication reporting date for each student recommended for a reader or scribe.
Braille and Large Print Exams
- The form Students Receiving Adjudication (Appendix 4A and 4B)[124 kb]
must be completed and sent to the Ministry by the adjudication reporting date for each student who requires either a Braille or Large Print Exam.
E-Exams (web-based)
- E-exams are available at the e-Assessment website .
- Students with special needs, who will be writing adapted E-exams, should logon to the Secure Exams Special Needs Format. Students who qualify for a spellchecker or Text Reader may access these adaptations when they logon to the Secure E-exams.
- It is recommended that students do a practice E-exam prior to writing a Secure E-exam. Practice E-exams are available with spellchecker and Text Reader at the website.
- If a provincial exam is not available electronically, students may use a word processing document for their open-ended responses. The students' responses must be printed and pasted or stapled under each question of the exam booklet.
