Forms and Instructions

Except for requests for a reader or scribe, schools or districts are responsible for applying ministry-based adjudication criteria to determine which students are eligible for adaptations to exam. For students who require a reader or scribe, schools must submit a Request for Adjudication with supporting documentation to the Ministry for decision. Separate forms have been provided at the Grade 10/11 and Grade 12 levels.

If a student requires a braille, large print, printable (PDF) or e-exam, the school must submit a Request for Special Format Exams:

Schools must submit the following forms to the Ministry on two separate Reporting Dates.

The form, Students Receiving Adjudication (Intended), identifies students who have met eligibility criteria for adaptations and who intend to write provincial exams. The form, Students Receiving Adjudication (Actual), identifies students who actually received adaptations to exam conditions which were used on provincial exams.
