Post-Secondary Institutions Choices Form

Students can provide their transcript information to a number of post-secondary institutions by filling in a Post-Secondary Institutions Choices Form [64 KB] in March. If students miss the form submission deadline in April, they should discuss next steps with their school counselor while school is still in session.
