Multiple Forms in the Graduation Program Examinations

  • Rationale for Using Multiple Forms

  • Beginning in January 2006, selected Grade 12 examinations will have multiple forms in major administration sessions (e.g., English Form A, English Form B). Many Grade 10 and 11 examinations are already administered in this fashion. This change, in conjunction with examination security, will greatly enhance the development of the item bank. In turn, the ministry will be able to:
    • equate examinations from one administration to another
    • minimize the effects of differences in examination difficulty
    • consider examination administration schedules that are more flexible and/or more frequent
    • enhance electronic delivery of examinations

  • Why do I need to fill in the Exam Booklet Form Code?

  • If your exam is returned to the ministry WITHOUT the exam booklet form code being filled in, the ministry will NOT be able to mark your exam and you will receive NO exam mark.

    Please make sure that you hand in your response booklet and/or answer sheet to the exam invigilator with the exam booklet form code CORRECTLY FILLED IN, otherwise there will be no way for the ministry to determine which exam booklet form you wrote and you will receive NO exam mark. If this occurs you may need to write again in another exam session. Please also ensure when you sign the signature sheet that you circle the appropriate form code.
