Has a Student Lost a Passport Award?

There is a $25.00 fee for each Declaration form issued, per student (regardless of the number of lost stamps). The $25.00 must accompany the request form from the school.

Steps you must take:
	If a student reports a lost stamp(s), the school must complete the request form (PDF, 58KB) and send it to the Ministry.
	There is a $25.00 administrative fee, which must accompany the request form, which can be paid by cheque (Minister of Finance), money order, VISA or MasterCard.
	The Ministry of Education will send a formal "Declaration of Lost Stamps" letter to the school Principal. The school and the student will both complete and sign the formal letter.
	The student will use the original "Declaration of Lost Stamps" form letter as the replacement for the Passport booklet and stamps.
	The student can redeem the "Declaration" the same way as a Passport booklet. Please see the student procedures information (PDF, 30KB) for details.
